I hereby agree to the following:
1. I affirm that I have answered all questions on my registration form honestly and fully disclosed all the information requested.
2. I acknowledge that by submitting the registration Typeform I commit to participating in an Ayahuasca Retreat with Nicolas Canon and Ayahuasca Colombia and to informing Nicolas Canon and/or Ayahuasca Colombia if I cannot join the selected dates (at least 30 days in advance). I understand that given the nature of this retreat, there will be no refunds. In case of a desire to reschedule the money paid will be prorated or transferred and used towards participation in another retreat (within 12 months of the original dates). Payment in full will be used as payment in full for future versions.
3. I understand that given the covid19 situation, the retreat dates may be changed in relation to the Colombian government's international flights policies and I will inform Nicolas Canon and/or Ayahuasca Colombia should I not be able to make the new dates assigned. Funds will then be moved to a future date in accordance to point 2.
4. I recognize that I am participating voluntarily in this retreat offerred by Nicolas Canon in collaboration with Ayahuasca Colombia in which I will receive information including about but not limited to health, meditative practices, interpersonal relationships and plant medicine. My decision to participate in this event is a personal decision. No one has made any promises or warranties as to the results or benefits I may receive or as to any specific results I may realize from my participation in this retreat.
5. I am committed to participating fully in this event for the full duration. I understand that this includes being present for all sessions. I understand that I am responsible for notifying a facilitator before I choose to leave the retreat without completing it.
6. I recognize that some of these practices require physical exertion that may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved. I understand that this event is not therapy or psychological counselling and is not a substitute for the treatments or services ordinarily provided by health care professionals for physiological or psychological complaints. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in the practices offerred in this retreat. I understand that the information and or practices offerred are not a substitute for the treatments and or services ordinarily provided by health care professionals. If I need or desire medical treatment, therapy, or psychological counselling, I will seek it from a licensed provider. I am well physically, psychologically, and emotionally, and commit to being responsible for my own well being during the course of this experience.
7. I understand that this retreat is for my personal benefit only and may not be appropriate for others.
8. I assume all risk associated with participating in this retreat and release Nicolas Canon, Ayahuasca Colombia and the retreat leaders and assistants, and shall defend, indemnify, and hold them harmless from and against all liabilities, claims, actions, losses, causes of action, and costs in any way arising out of my participation in the retreat. I, my legal heirs or legal representatives forever release Nicolas Canon, Ayahuasca Colombia and the retreat leaders from any liability for death, injuries or damages known or unknown due to their negligence occurring now or in the future, during or after my participation in this class, workshop, event, or training.
9. I understand that all payments made towards this retreat are non-refundable and by submitting the Registration Typeform I have read and accept all conditions and I am agreeing to pay the full amount of the retreat. Again, due to the limited amount of spots, there are no refunds. If I withdraw from the retreat for any reason whatsoever, no part of the event fee shall be refunded and any unpaid portion thereof shall become due and payable immediately from me to Ayahuasca Colombia.
By submitting the registration Typeform I have read and understand this agreement fully and intend to be legally bound by it. I accept the terms and conditions expressed in this agreement.
