For any change to a routine dismissal plan, parent/guardian must submit this form on the day of student dismissal change. Please note that early dismissal must take place by 3:35PM. Students cannot be picked up from KidZone or main office between 3:35-3:55PM. 
Dismissal changes must be submitted NO LATER THAN 3:00PM.
