Please complete this application to apply for Great Hearts Lakeside financial assistance with school supplies and uniforms on behalf of your child/children for the 2025-2026 school year. Please list the name of each child in your family who is currently registered to attend Great Hearts Lakeside in the 2025-2026 school year. This includes students who are already enrolled in the 2024-2025 school year.
Note: Due to the deadline for placing orders with 1st Day School Supplies, any applications received after June 3, 2025, will not be eligible to receive school supply assistance. If approved, one grade level kit will be provided for each enrolled student included on this application. Uniform vouchers will be available only until supplies run out.
School Supplies will be delivered to the school and available for pickup during "Meet the Teacher" (Lower School) or at the "Back to School Event" (Upper School) in August.
Uniform Vouchers: You will receive an email with instructions on when and where to pick them up.
