Please complete the following information if you are a registering a new student in the Gallatin R-V School District. 

If your student attended Gallatin R-V in the 2023-24 (PK-12) school year or have completed PK evaluation at the end of last year, please do not complete this form. Please use the re-enrollment form in your Lumen parent portal.

After your submission is processed, you will receive an invitation link from Lumen to register your student(s). District front offices will re-open on August 1, 2024. Please call the district (660-663-2171) or email
enrollment@gallatin.k12.mo.us if you have any questions regarding enrolling your new student. 

Additionally, we ask you to prepare the following documents and return them to the school:

-Proof of Residency

-Parent Photo ID

-Student’s Immunizations

-Student’s Birth Certificate

-Student’s Social Security No.
