FAE Tutoring & Intervention Center: Terms & Conditions1. Diagnostic Assessment & Academic Commitment
I understand that all K-12 enrollments begin with a mandatory Diagnostic Assessment to identify specific learning gaps. Tutoring plans are structured on a per-semester basis (Fall: August–December / Spring: January–May) to ensure consistent academic progress. I agree to support the customized learning plan developed for my child based on these results to ensure optimal growth throughout the semester.
2. Attendance & Cancellation Policy
Regular attendance is required to maintain consistent progress. Parents/Guardians must notify the center at least three (3) hours in advance of any session cancellation to avoid penalty. Cancellations made without adequate notice will incur a $25.00 Cancellation Fee to compensate the educational specialist for their committed time. Make-up sessions are subject to availability and director approval.
3. Diagnostic Fee, Rates & Billing Terms
I understand that the $35.00 diagnostic and consultation fee is non-refundable and is required to secure my child’s initial intake appointment. Tutoring sessions will not be scheduled until after the diagnostic assessment and consultation have been completed.
Standard Weekly Rate ($40.00/hr): Billed weekly and due on the Friday prior to the upcoming week of service.
Preferred Monthly Rate ($38.00/hr): Billed as a fixed monthly installment due on or before the 1st of each month during the semester.
Time of Service Requirement: Services must be paid in full prior to the start of each session or billing cycle. Payments received after the scheduled start time or due date will incur a $5.00 Late Charge.
Placement Security: Accounts must remain in good standing to maintain the student's recurring slot on our semester schedule. Unpaid balances may result in temporary suspension of services or forfeiture of placement.
4. Restorative Behavioral Standards
FAE is a restorative learning environment. Students are expected to adhere to community standards of respect, safety, and engagement. FAE utilizes the Restorative Learning Model (RLM) to resolve conflicts, manage academic anxiety, and build character. Continued enrollment is contingent upon the student’s cooperation with these standards and parental support of the restorative process.
5. Media & Work Product Release
I grant permission for Favor Academy of Excellence (FAE) to use photographs, video recordings, or digital images of my child for educational, celebratory, or promotional purposes. I also grant permission for the display of my child’s academic work products to highlight student achievement
