Living The Mission

The Living the Mission Program celebrates PYHIT staff who go above and beyond their everyday responsibilities, demonstrating compassion, integrity, teamwork, and dedication to the people and communities we serve. This initiative exists to highlight individuals who truly embody PYHIT’s mission in their daily work and positively influence those around them.

We invite you to help recognize these efforts by submitting a nomination. Anyone may participate, including staff, residents, family members, and community partners. If you have witnessed someone making a meaningful impact, supporting a colleague, showing exceptional care to a client or resident, or simply stepping up when it mattered most, this is your opportunity to acknowledge their contribution. Please use this form to share why the person you are nominating exemplifies Living the Mission.

Your nomination helps us shine a light on the outstanding work happening across PYHIT every day and ensures that excellence and dedication do not go unnoticed. If you would like to read more about "Living the Mission" please visit our website by clicking here
