The fee is due in 48 hours after the submission of this form. The Duplicate Diploma/ Duplicate Graduation Package shall not be processed without the payment.

If applicable payment is not received within 48 hours after the submission of this form, the Duplicate Diploma/ Duplicate Graduation Package request shall be automatically discarded.
