3rd Commonwealth Digital Health Awards 2018 - Terms and Conditions
This agreement is entered into by and between the organizer, Health Informatics Society of Sri Lanka whose principle office is located at the 6, Wijerama Mawatha, Colombo 00700, and the applicant for the 3rd Commonwealth Digital Health Awards 2018.

As part of your submission, you are required to read the following terms and conditions carefully and to attest by clicking on the ‘I agree’ button located at the bottom of your screen.

By pressing the ‘I agree’ button, you certify that you accepted the following terms and conditions.

Privacy statement
Any personal data submitted and intend to be submitted for the purpose of the competition will be used throughout the evaluation process. Further, submitted personal data may be shared between the organizers and any one helping with the administration of the competition. Retrieved personal data will be treated confidential and will only be used in furtherance of Commonwealth Digital Health Awards.

Intellectual Property rights
By submitting an entry for the 3rd Commonwealth Digital Health Awards and accepting any award, the applicant declares that he or she complies with the terms and conditions including that his or her submission does not infringe any intellectual property rights of any third party. Further, the applicant warrants that the submission is original and has been legally obtained and created, as well as is free from and infringement of any third party intellectual property rights.

Liability
To the fullest extent permitted by applicable law the Organizer’s, the Health Informatics Society of Sri Lanka exclude all liability for any direct, indirect, incidental or consequential loss or damage, including without limitation, any claims, actions, suits, settlement, judgments, cost, tax, charges or expenses, arising out of, resulting from, or in any way related to applicants’ participation in the Digital Health Sri Lanka 2018 and the 3rd Commonwealth Digital Health Conference and the 3rd Commonwealth Digital Health Awards 2018.

Further, the applicant shall indemnify for all damages and losses, cost, expenses, including the cost for legal actions, settlement, judgments suffered or incurred by the organizers arising out of or in connection with,

1. any third party claims made against the organizers as a result of any breach of terms and conditions by the applicant,

2. any actual or alleged infringements of third party intellectual property rights arising out of or in connection with any entry or submission made by the applicant to the Digital Health Sri Lanka 2018 and the 3rd Commonwealth Digital Health Conference and the 3rd Commonwealth Digital Health Awards 2018.

3. any claim made by third party against the organizers for death personal injury or damage to property arising out of or in connection with applicant’s conduct and output in connection with the Digital Health Sri Lanka 2018 and the 3rd Commonwealth Digital Health Conference and the 3rd Commonwealth Digital Health Awards 2018.

Confidentiality
It is understood and agreed by the applicant and the organizer, the information about the submissions and all entries including personal information to Commonwealth Digital Health Awards 2018 to keep confidential and not shared with 3rd parties, except to the extent needed for processing the competition of Commonwealth Digital Health Awards and in furtherance of Commonwealth Digital Health Awards.

Applicable Law
The agreement shall be governed by and construed in accordance with the laws of Sri Lanka. Any dispute or claim arising out of or relating to the terms and conditions shall be brought exclusively in the courts of Sri Lanka.
