25 Winter  Student Athlete Transportation request
Athletes can be transported by an adult over the age of 18 with parent/guardian permission.  The transporting adult cannot be a currently enrolled student. This form must be submitted AT LEAST 24 hours prior to the contest in which you are requesting to opt out of school transportation.

If the District incurs any of these types of expenses, I agree to reimburse the District.
