Applications should be completed by the trip participant, if age 18 or older, or by the parent or guardian of the trip participant if the participant is a minor under age 18.

Please complete this Transforming Trips application and submit it for our records.  Please take time to read the bullet points below and ask any questions you may have before completing the form.

→ Non-refundable deposit
Within a few days of submitting your application, instructions on how to pay the deposit for you and any minors included on this form will be emailed to you. Payment is due at that time.

→ Background Check
Once we receive your deposit a link to complete the background check will be sent to the email you provide on your application. This requirement must be completed by those age 18 or older (as of the
application date) in order to secure a place on the team. Only background checks from Good Hire Background are accepted and are good for up to three years.

→ Personal Campaign Page
If you wish to have a personal link created from our website, one will be provided to you after receiving your application.  You can share the link to let your family and friends know about your trip as well as ask and track donations.
