Waiver Statement

I give permission for my child to take part in the All-Sport Academy after-school program. I understand that activities may involve some risk of injury. I will not hold staff, or volunteers responsible for any accidental injury or illness that may happen.

If there is an emergency, I allow camp staff to get medical help for my child. I understand I am responsible for any medical costs.

I also give permission for photos or videos of my child to be used for camp purposes, unless I notify the camp in writing.
