FUUSN Religious Education Registration 2018-2019
We are so grateful to have you in our FUUSN community! Please share with us information about your family, children, and youth so we can best support you.

Registration forms currently accommodate two parent/guardians, though we understand that families come in multiple shapes and sizes. Please be in touch with us if the parents/guardians are not the primary people taking the child(ren) to FUUSN or if there are family circumstances of which you would like us to be aware.

If you have more than one child, you don't need to fill out the non-mandatory information about the parents/guardians the 2nd, 3rd, 4th, etc. time you fill out the form. (It won't autofill, but we can copy/paste parent info in our spreadsheet from first child to all additional children). You can simply type the any character (e.g. "asdf").

Supporting Our Cooperative RE Program - Parent/Guardian #1
A cooperative children’s Religious Education model means that the families whose children are in the RE program take the lead in teaching the RE classes. Other members of the congregation may teach as well, but the bulk of the teachers come from the participating families. ​Typically, parents are expected to teach an RE class or contribute in another significant way (~15 hours) to RE every other year​.

It takes YOU to make our RE program work. Check all that apply, including opportunities you've committed to or are interested in. FUUSNites working directly with children must have been attending FUUSN for more than six months and be known to the community. We want you to spend your first 6(+) months getting to know us and for us to get to know you! We run CORI background checks on all volunteers.
