Welcome to 2025 Cheer Club Cheerleading Registration! We are very excited that you have decided to join our family for basketball cheer season! Our goal for this season is for cheerleaders to gain cheer skills, build our confidence, teamwork and most importantly have fun! Cheerleading in our program is not just about cheering at all cost. Our program is about the experience. My job is to make sure that while participants are putting in time and effort into the cheer program that it is educational and fun. This will be an experience that they can keep with them for the rest of their lives.

CHEER RULES- 1. Cheerleaders will Always Pay Attention and listen to their Coaches. 2. All cheerleaders will always show good teamwork, cooperation and commitment. 3.Cheerleaders must wear squad practice gear and mask to all practices. 4. No gum chewing and no jewelry will be worn at practice, games or events. 5. Cheerleaders will arrive to events at least 15 minutes before  start time. 6. Cheerleaders will treat teammates and coaches with respect.7 All cheerleaders will be responsible to keep their practice gear &  uniforms clean at all times. 8. All practices, games, events and fundraisers are mandatory to attend. 9.Each Cheerleader is expected to follow the squad rules and guidelines. 10.Cheerleaders will not be allowed to be on phones during practices or games. Phones will be collected at the beginning of practice/ games and given back at the end of practice/ games. 11.Cheerleaders will not participate in any negative gossip or communication that may adversely affect PZCLA. This includes but limited to: verbally to other individuals, email, face book, twitter, Instagram, chat rooms or any other websites.12 Any physical altercations or bullying may result with suspension off the team or termination off the program participation. The above rules will be enforced by the head coach, assistant coaches and Team Managers. Any violation of the cheer squad rules could result in termination of the program participation. Medical Liability Release - I as parent or legal guardian give permission for my child or children to participate in the PZCLA program. I give permission to the hospital/ paramedic staff to administer immediate treatment to my son/daughter should he/she become ill or injured. In addition, I waive and release of PZCLA, coaches and volunteers from any and all claims and causes of actions arising from concerning any injuries, illness, losses or damages of any kind which my child or children or I may have as a result of or in connection with my child’s participation in the cheerleading program, and /or travel to and from the games, practices and events. PARENT AGREEMENT -CODE OF CONDUCT -Our son/daughter has our permission to be a cheerleader and participate in all cheerleading related activities with PZCLA for the upcoming season. I  have read and discussed with our child the rules, responsibilities and commitments of being a cheerleader. We understand the responsibilities and commitment of being a Cheerleader. I also understand the commitment to practices & virtual practices are very important. I understand that schedules, day, time or location maybe subject to change and that I will be notified of the change.  I also understand the role I  must assume as a parent of a cheerleader and I will assist in every way to see that the rules and regulations are enforced. 2. I will not use any inappropriate language and will behave in a manner that respects myself, PZCLA, parents, cheerleaders and the sport. 3. I  will support the coaches and the director in decisions made throughout the season. 4. I will not be involved in any matter of disrespect towards any members of the PZCLA coaches, parents or cheerleaders. 5.  I will not participate in negative gossip or communication that  adversely affect Pink Zebra Cheer. This includes but not limited to; verbally to other individuals, email, Facebook, Twitter, Instagram, chat rooms, or any other websites.   6. I will oversee that my cheerleader does not participate in negative gossip or communication that adversely affects PZCLA. This includes but not limited to: verbally to other individuals, email, Facebook, Twitter, Instagram, chats rooms, or any other websites. 7.  I will not create any social media accounts under any reference to PZCLA or any other gym at anytime. 8. I understand that the PZCLA and coaches assume no responsibility for any accidents or injuries that might occur either at school, practice, or an event away from school grounds at any time. We understand that if an accident or injury may occur, I am responsible for taking my child to seek proper medical attention. 9.  I understand that coaches have the right to suspend any athletes or parents participation in activities as disciplinary action all the way to removal of the program. 10.  I understand the time commitment involved with this activity and  if our child is dismissed or quits the cheerleading squad we will not receive any refunds. 11.  I  understand that PZCLA will place  athletes on a team based on what is best for the athlete and the program and I will accept the decisions of PZCLA coaches and its volunteers as final decisions. I also understand that it is sometimes necessary to move athletes from one team to another during the season to help the athlete program. 12. I understand the importance of punctuality and  that participation in practices, camps, games, spirit events is mandatory. Games fall on Saturdays. 13. I  do hereby give my consent and support for our child to be a cheerleader for PZCLA for the upcoming season and I will conduct myself with respect and integrity, displaying positive sportsmanship at all times. 14. I understand that there is a dropout fee in the amount of $200 should I decide to remove my cheerleader from the program for any reason.  There are no refunds for the program at any time. 15. I understand that if I drop out of the program at anytime, all discounts, specials, payment plans and scholarships will reverse and I will payback any discounts, specials and scholarships given, additional fees and any remaining cost of the program. All discounts, specials, payment plans and scholarships are contingent on completing the full season ( last day) of the program. Payment is due within 30 days of dropout. 16. DROP OUT FEE- Parent understands that should you for any reason want to remove your cheerleader(s) from the program , there will be a program drop out fee of $200 per cheerleader and all discounts, specials & scholarships received will be reversed and I will pay any remaining balances on my account. If for any reason your cheerleader(s) are dismissed from the cheer program, all discounts, specials and scholarships will reverse and parent will payback discounts, specials and scholarships and is still be responsible for paying $200 drop fee per cheerleader and any remaining balances on the your cheer account. SOCIAL MEDIA-Cheerleaders are expected to maintain appropriate behavior on social media sites. Coaches will be monitoring social media sites during the season ; if a post/ tweet/ picture is inappropriate, a coach will ask the cheerleader to remove the post. Refusal to remove the post or repeated offenses will result in parent, cheerleader, and coach meeting to discuss further action. Initial below

PAYMENT/ FAMILY PAY PLANS-Parents understand that the full cost of cheer is due at the beginning of the season at sign up or registration. As a courtesy we offer our families monthly family pay plans to help spread out the cost of cheer during the season. There will be an additional $25 charge for all family pay plans. Parents understand that should you drop out before the season ends that you will still be responsible for paying a drop out fee of $200, any remaining balance and any other additional charges that may apply. Any remaining balances after the season ends or drop out of 10 days past due will be sent off for collections. There will no exceptions.- Initial below

Credit Card Agreement: Parents understand that by choosing to pay family pay plan with scheduled fees, you agree that a credit card or debit card information will be kept on file. If schedules fees are not paid on scheduled day, a $25 late fee will be added to the balance and credit card or debit card will be charged on the following day. Initial Below

LATE FEE- A credit or debit card will kept on file for cheer families on Family Monthly Payment Plan.  Payments and tuitions are due on or before each month on a set date. A late fee in the amount of $25 plus payment due will be charged to the debit or credit card if payment is late. Initial Below

PHOTO RELEASE- I give my permission to PZCLA and its volunteers or coaches to take photos and video’s of my child. I also give consent to PZCLA and it’s coaches to use pictures and/ or video of my child on the PZCLA and its league websites, press release articles or social networks. Please initial below

PRACTICE GEAR & UNIFORMS- I  understand that my cheerleaders uniform is rental and I will do my best as a parent to help my cheerleader(s) keep thier practice & performance uniform in a place where they can find it,  free of any damages and clean for all team performances during the season. I also understand that at the end of the season, my uniform must be returned on the scheduled date with no damages. There are additional charges for unreturned skirts & any damages to cheer rental items.  Initial below

CHEER PRACTICE GUIDELINES- ☆All cheerleaders must check in at the beginning of practice with thier coach or team mom. ☆Parents, family members and friends (spectators) will not be able to sit in practice to watch. ☆ Wear Proper Attire – Workout Clothes, Soft Soled Shoes ☆Take a shower and wash all clothing after each practice ☆  Sanitize hands after each practice ☆ Athletes Tie Hair back slick and away from face. ☆Bring Water Bottle Clearly Marked with Name. ☆Parent understands that there will be no snacks during practice. ☆Practice Includes: Stretching/Flexibility, Basic Conditioning, learning new cheers and chants and learning a new cheer dance routine.   PLEASE INITIAL BELOW.

PZCLA is taking steps to reduce the spread of COVID-19; however, PZCLA cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending PZCLA activity could increase the risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending PZCLA activity and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 may result from the act, omission, or negligence of myself and others, including, but not limited to, PZCLA,  volunteers, and other participants and their families. I voluntarily agree to assume the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or mychild(ren) may incur by reason of PZCLA activity (“Claims”). On my behalf, and on behalf of my children, I hereby release and covenant not to sue PZCLA, its affiliated organizations, employees, volunteers, agents, and representatives, of and from the claims. ACKNOWLEDGEMENT: I understand the cost of cheer financial obligation, payments, family pay plans & responsibility associated with joining PZCLA Cheerleading or recreation programs.  This application includes cheerleader(s) of the same household. I will participate in all required fundraisers, and pay any balance that I do not fundraise. ATTENDANCE: Cheerleading is a team sport which requires every cheerleader to know their responsibilities and perform as part of a TEAM. We are unlike any other sport in that we do not have substitute members if someone is absent. Attendance is essential to the success of each season. Attendance will be taken at the beginning at each practice. Each Cheerleader’s 100% commitment is required as each position on the squad is dependent on the other. When one Cheerleader is absent, it places hardship on the entire squad and can become a safety issue. It is for these reasons the Cheerleader and the parents must be advised of the importance of compliance to the attendance policy. Initial below *
