By completing this questionnaire, it is expected that volunteers have read the volunteer handbook and completed the self guided training session. The questionnaire must be completed once every five years, as does the Criminal Record Check and Vulnerable Sector Screening. During years when a new check is not mandatory, an offence declaration must be completed.

Communication with assigned staff is important because: *

it helps the volunteer understand classroom/school policies

it allows the volunteer and staff member to clarify responsibilites and task modifications

it provides the opportunity to discuss feeling about the way tasks are performed

all of the above

The following is NOT a method of success for a volunteer: *

being a positive role model

allowing student discovery

following the teacher's lead

yelling at, or embarrassing a student

As a volunteer, confidentiality is imperative, but volunteers MAY share the following information: *

a child's academic performance

if a child has an IEP

a staff member's relationship status

a fun activity that students completed in class

As a volunteer on a trip/excursion, I am responsible for: *

confirming how I will be supporting the teacher

knowing and understanding the expectations of the teacher

knowing the names of the students in my small group, with a list of them, should I have some students to supervise

being punctual to meet the bus

all of the above

If a fire drill occurs at school, I: *

have free time as a volunteer

run quickly out of the school and away from the building

leave the building via the exit route posted in the classroom with the group of students or class and if necessary join the classroom teacher at then designated meeting area

all of the above
