This form is to request a change in learning for your scholar for Distance learning to Hybrid (scholars attend in-person 2 days a week) or Hybrid to Distance Learning ONLY. Parent/Guardian will be notified via phone once the request has been approved. A change can take up to two weeks to be approved to ensure a safe transition. ONE SCHOLAR PER FORM PLEASE!!
