This form is to assist us with planning and preparation regarding the 2nd semester of the 2020-2021 school year.  

 If you choose traditional, please check the box marked traditional.  The following is expected of traditional students:
-Attend school on time daily when the student is not quarantined.
-Complete all required assignments to include classwork, formative/summative assessments, etc.

If you choose the virtual option and your application is approved, the following is mandatory:
-Attend all virtual zoom lessons daily and following class schedule.
-Complete all assignments Schoology and schedule time with teachers if additional assistance is needed.  
-Take all high stakes assessments face-to-face such as ACT/w writing, ACAP, ACAP Alternate, ACCESS, etc.
-Email or call teachers if assistance is needed during teachers' office hours.
