CONSENT & LIABILITY POLICY:
Client gives consent to participate in a beauty service by The Lovely Artistry Collective and/or its affiliates or contractors. Client understands the following:
a. Per our liability insurance coverage, Client understands that there is no touching an artists tools or products for any reason. If an artists supplies or products are touched by Purchaser or anyone from that party they will owe full retail value of said product at that time, and services will not continue until payment has been received.
b. To remain in line with requirements of liability insurance and to maintain ensure proper sanitation, quality of work, and high performance of the products, Artist(s) only use products and brushes from their professional kit. We cannot guarantee the outcome of using a client’s personal makeup or ensure there isn’t cross-contamination exposure. In the event a client has a severe allergy and must use specific products approved by their doctor, our Artist(s) will use their own application brushes to apply the products and The Lovely Artistry Collective nor Artist(s)s will be held responsible for any adverse reactions, cross-contamination responses, etc.
c. Artists of The Lovely AC or any contractors of The Lovely AC cannot be held liable for any allergic reactions or any other reactions that could arise from this class/service.
d. While the application of makeup is generally regarded as safe, that allergies, or reactions to makeup products (such as latex) can result in injury.
e. Client further agrees to release the Artist/companies from any liability from all skin complications due to allergic reactions. The Lovely AC and any associates are not held responsible for complications that may result after leaving the facility.
f. If client has any adverse reactions to any makeup products, client agrees to seek immediate medical treatment for such adverse reaction.
g. Client understands and agrees that once they leave chair and services are complete, they agree they are happy with services and did not leave unhappy with their results.
h. The Client understands The Lovely AC reserves the right to refuse services if or alter services if unsanitary conditions exist and/or contagious infections are present. Clients understand and acknowledge that Artists may alter service or NOT perform services where any contagious infections such as pink eye (conjunctivitis) or cold sores/fever blisters are present and there shall be no refund of any monies paid. Purchaser acknowledges and agrees that verbal abuse toward a Artist/Stylist is unacceptable by any person at the event and shall be grounds for the Artist/Stylist to cease services immediately. Client acknowledges that they will not be entitled to a refund for any work already performed and purchaser may not review artist or stylist vendor poorly on social media or other review sites if this happens. Client also acknowledges that they will allow artist to work in safe conditions for personal safety. At any time artist or stylist feels physically threatened or verbally abused, artist/stylist may cease the performance of services and exit the building. This also includes having a smoke-free area to work as a smoking area or a place where an Artist is subject to be around smoke is not acceptable.
i. In an effort to make the day as smooth as possible for our brides, we share this policy with all those receiving a service: Per our policy, all clients receiving a beauty service are required to be at the get ready location at the agreed upon start time. A late start may result in fewer than contracted services rendered. The Artist will make every attempt to provide quality services for all contracted participants within the remaining time-frame but can make no guarantees. Artist is unable to extend the contracted ready by time due to a late start by parties lateness.
I have carefully reviewed policies and considered all risks involved in this activity, and hereby agree to hold The Lovely AC and their respective artists/assistants harmless for all personal injury which could result from participation in these services.
