Substitute Sick Leave Form
To use Paid Sick Leave, fill out this form with your information and the information of the substitute job that you are turning down. Press submit when you have completed the form.

-Substitutes accrue sick leave beginning the 30th day after hire.
-You may use accrued sick leave beginning the 90th day after hire.
-A maximum of 3 days of sick leave may be used each school year (24 hours for classified and 21 hours for certificated substitutes).
-In order to claim sick leave, you must have accrued at least as many hours as the job that you are turning down (substitute teachers must indicate 7 hours for a full day and 3.5 hours for a half day).
-Sick leave pay is based on the job that you are turning down.
