Some things to note before submitting this form: 

1.) If you've been blocked it most likely means you're not compliant with Google's TOS and AUP.  Please make sure you read through both links and are compliant.

2.) One of the most common issues we see is, attribution violation, which means:

Your application violates Section 3.2.2(b) of the terms, which requires developers to “display all attribution that (i) Google provides through the Services (including branding, logos, and copyright and trademark notices); or (ii) is specified in the Maps Service Specific Terms (https://cloud.google.com/maps-platform/terms/maps-service-terms/).” In addition, developers are not allowed to “modify, obscure, or delete such attribution.”

Please update your application so that the [“powered by Google” branding / Google logo / Google copyright and trademark notices] on the map displayed in your application is clearly visible as required by the Terms of Service.
