2020 Midyear Meeting Reservations
The 2020 Midyear Meeting will be on January 11th, at Saint Ambrose Catholic Church Social Hall (basement), 2315 East Redondo Avenue, Salt Lake City, UT 84108.

This form allows you to make reservations for breakfast and lunch (only). These meals will be provided at no cost for those that have made reservations. Deadline for reservations is January 3rd.

If you will be paying by credit card for the dinner, please make your reservation online at https://squareup.com/store/UT-KofC-Mtgs first, then fill out the form below.

If paying for dinner by check:
