We kindly ask that you submit one form per student. You can, however, give us notice for multiple dates in each form.

We thank you for taking the time to give us notice of your upcoming absence(s). Please keep in mind that unless you have a doctor's note or an emergency, 24 hours notice is REQUIRED for the class to be eligible for a make-up.

When a registered customer gives us notice of their upcoming absence, it allows us to use the empty slot for other students pending a makeup.

As per our policy, we have a maximum of 1 makeup every 6 weeks for private lessons, and 1 makeup every 12 weeks for group lessons. Please note they can not be rescheduled. This restriction is due to the limited amount of empty slots in our schedule and to be respectful of our instructors' time.

IF ELIGIBLE, a member of our staff will be contacting you via call and/or text about rescheduling your missed lesson once the the next availability appears on our schedule. Please note that makeups do not expire; however, due to our limited capacity in the schedule, it may take some time to get back to you.
