Polish Hospital Federation conference and discussion with Polish healthcare Authorities entitled: “Innovations in hospitals and healthcare systems – challenges for hospitals in Poland” in New University Hospital of Kraków.
Meeting with NEOHOSPITAL management and presentation of new technologies dedicated to medical procedures in practice (incl. Da Vinci robotic surgery).

I consent to the processing of my personal data provided in the form above by the personal data administrator, which is the Polish Federation of Hospitals ("PFSz"; address: Nowogrodzka 11, 00-513 Warsaw) for the purposes of organizing the JKP 2019 conference. The basis for data processing in the above the purpose is art. 6 clause 1 lit. a GDPR, i.e. the consent granted and art. 6 clause 1 lit. f GDPR, i.e. the legitimate interest of the administrator. Data can also be processed when it is necessary to fulfill legal obligations incumbent on DZP, i.e. pursuant to art. 6 clause 1 lit. c GDPR. Data may be transferred to entities with the help of which PFSz accomplishes the indicated purposes, including entities maintaining IT infrastructure. Providing data is voluntary, but failure to do so will prevent registration for the conference. It is possible to withdraw consent to the processing of personal data, which, however, will not affect the lawfulness of the processing that was carried out on the basis of consent before its withdrawal. In contractual relationships, providing data is a requirement for the conclusion and implementation of the contract. It is possible to object to data processing, request access to it, rectify, delete, limit processing and transfer. The data is stored for objection or withdrawal of consent.
