The Dewitt Junior Panther Football and Cheer Program strives to provide the highest quality youth sports
program for its participants. The program has created Coaches, Player, and Parent Code of Conduct expectations to establish minimum behavior and performance standards.

Please use the following process when filing a complaint:
1. Speak to the coach privately and in person. We urge you to wait 24 hours after the incident unless it is a case of abuse or criminal neglect. This step resolves most disagreements.
2. If you are not satisfied with the coach’s response or do not feel comfortable speaking with the coach, please
submit this form to the DJP Board for review.
     -- Complaints will not be acted on unless a written statement of the complaint is provided via this form. The DJP Board may contact you for additional information and/or attendance at a meeting.
     -- This form is submitted to the DJP Board's Discipline and Complaint Committee, which will review, help resolve, and/or advance the issue to the full Board
