2) Confirming and acknowledging the following 5 statements and that you would be able to produce documentation on request:

(A) The business has completed a Risk Assessment for the event and a copy has been sent to the Event Manager.

(B) Equipment used during the event conforms to Health and Safety Legislation and that any electrical equipment has a current PAT test certificate/stamp.

(C) The business holds Public Liability Insurance to the value of at least £5,000,000.

(D) The business conforms to Food Hygiene (England) Regulations 2013 and has food safety management procedures in place with a Food Hygiene Rating of 4 or above

(E) I agree to sell only the items detailed on this application form
