Relax & Read Subscription Box Request Form
Youth ages 10-18 years old can register to get a once a month subscription box full of fun stuff to help them relax and de-stress by filling out this form. Boxes will be available January through May 2021.

What's in the box?
1.) A calming, fun activity with all the supplies included.
2.) A library book, already checked out to you, chosen for you based on your answers to questions on this form.
3.) A special treat such as tea, candy, popcorn, etc.

When and how do I get my box?
We will contact you when your box is ready to pick up. You can pick it up at the library via the service window (if we are curbside only). When you are done with the book, return it to the book drop!
