This event is open to TRIO students!  Lunch & transportation provided.  Check in will be Friday February 16th @ 8:00am in room S203 TRIO Office Suite (Centretech Campus, Student Centre Building, 2nd Floor) with plans to return to campus by 3pm (roughly).

Risk Waiver and Release Agreement

1. I have secure health insurance to provide adequate coverage for any injuries or illnesses that I may sustain or experience while participating in said event/activity, including but not limited to costs related to medical care or evacuation. By my signature below I certify that I have confirmed that my insurance coverage will adequately cover me while I travel, and hereby release the State of Colorado, the State Board for Community Colleges and Occupational Education, the College, and their officers, representatives, employees and agents (“Releasees”) from any responsibility or liability for expenses incurred by me for injuries or illnesses (including death) that I may incur because of those injuries or illnesses.

2. I understand that students are representatives of the Community College of Aurora when they participate in a college-sponsored trip or off-campus activity. Students’ behavior on TRIO-sponsored trips or at off-campus activities reflects on the College and therefore students are expected to conduct themselves appropriately and comply with applicable College policies, procedures, rules and regulations. Violation of these requirements may subject students to disciplinary action and/or expulsion from the event/activity.

3. Alcohol/Narcotics Prohibited. The use, being under the influence of, manufacturing, possession, distribution, purchase, or sale of alcohol or other drugs while participating in this event/activity or at any function authorized or supervised by the Community College of Aurora is prohibited. Pursuant to federal law, the use or possession of marijuana continues to be prohibited while a student is at any function authorized, supervised, or made aware of by the College.

4. I understand that although the College has made every reasonable effort to assure my safety while participating in this event/activity, there are unavoidable risks in any type of travel, and I hereby release and promise not to sue the Releasees from and against any and all liability for any harm, injury, damage, claims, demands, actions, causes of action, costs and expenses of any nature that I may have or that may accrue to me, arising out of or related to any loss, damage or injury, including but not limited to suffering and death, that may be sustained by me or any property belonging to me, whether caused by negligence or carelessness of the Releasees, or otherwise, while in, on, or in transit to or from the off-campus event/activity. It is my express intent that this Agreement shall bind members of my family and spouse, if I am alive, and my estate, family, heirs, personal representatives, or assigns, if I am deceased, and shall be deemed as a release, waiver, discharge and covenant not to sue Releasees from any claim by me or my family, arising out of my participation in this event/activity. I further agree to save and hold harmless, indemnify, and defend Releasees from any claim made by me or my family, arising out of my participation in this event/activity.

5. I agree that should any provision or aspect of this agreement be found to be unenforceable, all remaining provisions of the agreement will remain in full force and effect.

6. I represent that my agreement to the provisions herein is wholly voluntary, and further understand that prior to signing this agreement, I have the right to consult with the adviser, counselor, or attorney of my choice.

7. I agree that should there be any dispute arising from or related to my participation in this event/activity that would require the adjudication of a court of law, venue will lie in the City and County of Aurora, Colorado, and the cause of action will be determined by the laws of the State of Colorado.

8. This agreement represents my complete understanding with the College concerning the College's responsibility and liability for my participation in this event/activity, supersedes any previous or contemporaneous understandings I may have had with the College on this subject, whether written or oral, and cannot be changed or amended in any way without my written concurrence.

9. I represent that I am at least eighteen years of age or, if not, that I have secured below the signature of my parent or guardian as well as my own.

Travel Health Information

Participating in off-campus events/activities are exciting and rewarding experiences; however, there is the chance that both physical and emotional demands of participation may involve certain health and emotional risks. Participating in off-campus events/activities may aggravate certain conditions, illnesses, or disabilities which you may be able to manage well under more familiar circumstances.

If you have medical or emotional conditions which you or your physician feel may affect your ability to participate in the event/activity and/or for which you may need accommodation, please advise the Accessibility Services Office of these matters as soon as possible, and in any case well ahead of departure so that these concerns can be addressed to assist you in having a successful and positive experience. This information will remain confidential and will be provided to the appropriate program staff in the event medical attention is necessary.

Student Travel Behavior Agreement

I will conduct myself in a responsible manner and in accordance with the Community College of Aurora’s Student Conduct Guidelines as specified in the Student Handbook. I realize that I am a representative of the Community College of Aurora and that I have been chosen by my organization to represent the College and its interests. I understand that any action I take will affect people’s opinion of my organization and College.

I agree to stay at the designated hotel and return via transportation arranged by the College. I will attend and participate in all aspects of the event/activity to the best of my ability.

I understand that I must give 7-day notice if I am unable to attend, or I must reimburse the organization and the College for any expense(s) that are non-refundable.

I will engage in behaviors that are responsible and mature. Intoxication, use of illegal substances, including marijuana, and disruptive, abusive, or inappropriate behaviors, and violation of conference, hotel, or College rules, may result in dismissal from the conference and potential disciplinary action by the College.

I understand that I am responsible for any financial charges that I incur that are outside the scope of the activity and as previously discussed in pre-departure meetings. For example, incidentals in the hotel.

If I am asked to leave the event/activity because of my behavior, I understand that I must reimburse the organization and the College for any expenses related to my participation in the conference. If conference, hotel, or College rules are violated during the conference, a record of the incident will be submitted to the responsible campus representatives. I agree to have an open mind, be an active participant during the event/activity, be respectful of others, and keep my cell phone put away during sessions.

I agree to communicate with my adviser if challenges, concerns, or issues arise.

Release Consent

I consent to and authorize the use and reproduction, by the Community College of Aurora or anyone authorized by the College, of any and all photographs/digital images/videotapes/recordings at the Community College of Aurora or on any trip or at any activity sponsored or supervised by the Community College of Aurora, and the right to copyright and/or use, reuse and/or publish or republish photographic pictures, digital images, videotapes and recordings in conjunction with participant’s name.

I also give permission for these photographs/digital images/videotapes/recordings to be used in their entirety and/or edited versions for marketing purposes by the Colorado Community College System.

Furthermore, permission is also given for the photographs/digital images/videotapes/recordings to be used by the Community College of Aurora at any time in the future without further clearance from me. I understand that the Community College of Aurora may use these photographs/digital images/videotapes/recordings for marketing purposes.
