Author/Publisher Purchase Request Form
Thank you for your interest in submitting a title to The Lane Libraries circulating collection!  This form should ONLY be used by authors or publishers requesting that their title be added to the library's circulating collection.  

If you are looking to see your title included in the Smith Library of Regional History (sml@lanepl.org) or the Cummins Local History Room (reference@lanepl.org), please contact them directly.

General patron requests for purchase should be entered on our Suggestion for Purchase form.
