NWGA Hope Over Suicide Coalition MENTAL HEALTH SURVEY
The NWGA Hope Over Suicide Coalition aims to reduce stigma and empower individuals to play a role in suicide prevention, to build community advocacy for those at risk or affected by suicide, and to build capacity for support services across the Northwest Georgia area.

This survey is an anonymous instrument created by Hope Over Suicide which was designed to bring understanding of the state of our citizen's mental health. We hope that you will be honest and please know you will not be contacted in anyway as a result of completing the survey. Please complete this if you are a resident of North West Georgia (Catoosa, Chattooga, Dade, and Walker). Please share the survey with others - the more data we collect, the better we can know how to help!

These questions are replicas of those used in the Georgia Student Health Survey, an anonymous, statewide survey instrument for grades 3-12, created by the Georgia Department of Education in collaboration with other state partners. The Coalition wants to be able to compare adult answers with existing student data. Utilizing those questions will produce consistency in our results to aid in analyzing our data.
