Please complete the following information to register your swimmer(s) for this season. Each swimmer's information can be listed within this form, one swimmer per section.  Emergency contact information, use of pictures and hold harmless agreements are included at the end of this form.  Payment can be made in the form of cash or check written to "OP Swim Team".  Payments can be mailed (OP Swim Team, C/O Kathy Callis 2412 Guilford Ave, SW, Roanoke, VA 24015) or dropped off at your first practice to the team drop box.  Kindly note "swim team registration" in the memo of your check or on the envelope when dropped off or mailed.   Registration and payment MUST be received prior to the first swim meet your swimmer will compete in.
