Please type your own full name *
Your answer
Please type the FULL NAME of your professional society (not just the abbreviation, please -- for example, I would type Greater San Diego Science and Engineering Fair, not GSDSEF), the way you would like to see it listed in our materials. *
Your answer
Sometimes, we need to telephone you with a quick question, although most communication is by e-mail. What telephone number should we call? (Please remember the area code).
Your answer
We would like a mailing address, in case we need to forward a thank you note or otherwise send something through the mail. Please remember the zip code.
Your answer
We would like to add a website for each Professional Society, so that students can find out more about your vision and mission. If you have one, please type it below.
What are the criteria you will use to determine which students win your awards? If you do not yet know, please say so. We will be happy to help you develop criteria.
Your answer
Please describe the planned award your winning students will recieve -- ribbons, plaques, gift certificate, checks, bonds, books, invitations to present at a meeting, publiucation, etc. In addition to a description, we like to know the monetary amounts. We do not share this information with students, but we do aggregate the data when we apply for grants to fund the GSDSEF. If you do not yet know, that's fine, too.
Your answer
How many awards do you plan to provide? If this is not yet determined, please say so.
Your answer
If you would like further e-mails to go to other individuals, or you want to provide another e-mail address in case of trouble with the one you provided above, please type the additional contact e-mail(s) below.
