If you are coming to Festival Cruïlla with children up to and including 15 years of age, you will need to fill this form before arriving at the festival.

When you arrive at the Parc del Fòrum, enter the festival through the FAMILY ACCESS, located in the leftmost lane. Remember, when entering the venue, different documents must be presented depending on who accompanies the child:

• In the case that the parent or legal guardian is accompanying the child, adults will have to present an ID card and additionally the original or photocopy of the official family logbook or legal documentation that confirms the relationship with the child.
  
  
• In the case that the accompanying person is not the father, mother, or legal guardian, adults should present an authorized signature from the parent or legal guardian, together with a photocopy of the parent or guardian’s ID card, and the child's and the adult companion's ID cards.
  
  
• In both cases, the person responsible for the child must sign this form releasing the Festival from all liability.
