All licensed Colorado facilities receiving trauma patients by ambulance or other means must submit an application in writing for designation or re-designation as a level I-V trauma center.  Those facilities not seeking level I-V trauma center designation must submit an agreement of non-designation to the Department. Use this form to start the process of trauma designation or re-designation. The department will contact the person named on this form to gather more information and set a review date.
