**PLEASE READ BEFORE COMPLETING**
It is the responsibility of the requester to notify the Facilities Coordinator of cancellations or changes to the facility usage no later than 48 hours prior to the event. Fees must be paid in full by the conclusion of the event however, in some cases, we may require prepayment. If your request falls into a "prepayment" category, we will notify you accordingly.
A submission of the Facility Use Application with applicant's signature serves as follows:
I accept responsibility for the building usage fee, supervision, damage & compliance with USD 402 BOE requirements & have read the Facilities Use Agreement.
Your facilities use request will be under review upon completion of the following form. You will receive notification of approval or denial via the email provided.
