Cooperative Reuse Waiver and Photo Release Form

I, [YOUR NAME], agree to the terms and conditions outlined in this waiver and photo release form in consideration of being allowed to participate in Cooperative Reuse (CR). 

  1. Assumption of risk: I understand that participation in Cooperative Reuse involves certain risks, including but not limited to the risk of personal injury, and other harm. I voluntarily assume all such risks.

  2. Release of liability: I hereby release, waive and discharge, the right to sue the managers and organizers of Cooperative Reuse from any and all liability, claims, demands, and causes of action related to loss, damage, injury, and death that is sustained to myself and any property belonging to me. 

  3. Photo release: I grant Cooperative Reuse organizers, volunteers, and employees the unrestricted right to photograph and publish photographs or videos of me taken during the Cooperative Reuse event for any lawful purpose including illustration, publicity, and web content. 

  4. (Minors only): If the participant is under the age of 18, I certify that I am the parent or legal guardian of the participant and I consent to the participant’s engagement with the Cooperative Reuse program and all the conditions stated in this agreement. 

  5. Community expectations: Participants in Cooperative Reuse must adhere to expectations to be respectful, collaborative and supportive of a sustainable and equitable move out. This includes:

    1. following the directions of CR organizers and campus staff;

    2. maintaining a respectful attitude towards other visitors, CR organizers and campus staff; 

    3. “stepping back, after stepping up” to ensure all participants can enjoy CR’s limited resources and free items; 

    4. following relevant campus policies, including paying for campus parking per campus policy; 

    5. Keep track of your trash: if you bring in food or other items that produce waste, you pack it out. CR is not responsible for sorting and disposing of your waste.

CR organizers can refuse service and ask participants to leave campus property if community expectations are not adhered to, and when necessary, CR organizers can and will call on campus security professionals, including UCPD and Parking & Transportation.
