In an effort to assist students to connect with friends outside of school to foster friendships we would like to offer a grade level directory. You may use this directory to contact students and families outside of school to make plans and/or invite to parties, playdates etc. If you choose to use this directory for party invitations - please follow our student handbook. Invitations may not be passed out at school (this includes before/during/after school, at recess or on the bus). This directory will only be shared with families who have given permission to participate. By completing this form you are giving permission for Applewood staff to share your information with other families.  Please fill out a form for each child in each grade level. If at any point you would like a directory for your child's grade level, please contact the teacher.
