Pre-registration for attending church service, July 12, 2020 - 10AM. Attendance is limited, and is available on a first registered, first serve basis.

By completing and submitting this application, you agree to adhere to the following guidelines:

1. Attendance is now open to both Philadelphia Church members and non-church members.

2. UPDATE: You will only be contacted if the list is full. Once you sign up, you are registered to attend. No confirmation will be given.

3. You are agreeing to attend at your own risk.

4. Children are now welcome.

5. Age restrictions have been lifted.

6. Chairs and pews will be clearly marked for use and non-use, and ushers will assist seating congregants, respecting social distancing guidelines (family members from the same household may be seated together).

7. Prior to and following any in-person service, our facilities will be sanitized.

8. Attendees are advised that, if they choose, they may wear masks and/or gloves.

9. Attendees are advised not to engage in hand shaking or other physical contact.

10. Hand sanitizer will be available for use throughout the facility.

11. Points of entry and exit will be clearly marked, and separated from each other to establish a one-way traffic pattern.

12. Doors will be propped open or held open by ushers to prevent the need for congregants to touch doors while entering and exiting the church or sanctuary. Doors close at 10AM, and no entry will be available once doors are closed.

13. Anyone with any symptoms of COVID-19 illness, and/or anyone who works on the "front lines" in healthcare facilities that treat COVID-19 patients are asked to forego our in-person gatherings.
