As required by the Archdiocese of Denver, Immaculate Heart of Mary Catholic Parish
obtains background checks for all of our volunteers. Please complete the information below so
that we can set up the request. PLEASE NOTE: this form is not the background check, it only provides info to set up the background check link that you will be receiving soon.

Within a few days, you will receive an email from our background
screening company, Fastrax. The email will contain a secure link for you to fill out the additional
information needed to run the background check. The link will expire within a couple days, so
please complete it upon receipt. It should only take about 5 minutes to complete. Thank you for
your cooperation and compliance.
