This instrument is checked out from the McKinney ISD for a $100.00 per school year usage fee. Please note that the usage fee will be $150.00 for two instruments. You must fill out a separate form for each instrument. Please make checks payable to Faubion Middle School or bring cash. This fee covers regular maintenance only, and does not apply to excess damage or abuse. Every effort has been made to provide the band students of McKinney ISD with a quality instrument in the best possible condition.  It is the student’s responsibility to take extra precaution and return it in the same condition at the end of the year. The student is responsible for performing regular cleaning and proper care of the instrument while it is in their possession. Any needed repairs should be brought to the attention of the directors as soon as possible to avoid any extra damage that may occur. The instrument should be transported in its case, and stored correctly in the appropriate place in the band hall. Students will be financially responsible for any damage not on this form upon return of the instrument.  If you have a home and school instrument, please fill this out 2 separate times.
