Regular exercise is associated with many health benefits, yet any change of activity may increase the risk of injury. Completion of this questionnaire is a first step when planning to increase the amount of physical activity in your life.
FRC invites you to try out evening club sessions for three weeks before deciding to join: that’s six sessions. If you wish to continue running with us after three weeks from today you must purchase club membership.

If I have answered Yes to any of the questions above I will discuss this with the run leader before my first session and it is my responsibility to ensure that they understand any limitations or constraints that may be required.

I will inform my Leader in Running Fitness if there are any changes in my circumstances. I take part in any recommended programme entirely at my own risk and waive any legal recourse for damages to myself or property arising from my participation.

If I have any doubts about engaging in physical activity I will consult with a doctor before starting activity. I understand that a doctor, and not a leader or coach, must be content before I start a programme of activity. Additionally, if my health changes during the programme I should refer to my GP or doctor for advice.

Privacy Statement:
Your responses to this form will be stored in a Google Sheets documents for three years whether you choose to join us or not. It is only accessible to those with a legitimate reason for access.
Your information will not be used for any purpose other than for running club administration.
