3D Tune-In: Hearing Aid Questionnaire
This questionnaire aims to gain greater insight into how well hearing aid users communicate with others in various situations. This insight will feed into the development of a range of games and applications that aim to help people with hearing difficulties learn how to use their hearing aids more effectively, and to overcome specific challenges they experience in everyday life.

The questionnaire will take 10-20 minutes to complete, and participation is completely voluntary and all responses are anonymous.
This questionnaire has been developed within 3D Tune-In (http://3d-tune-in.eu), a Horizon 2020 research and innovation action project, financed by the European Commission (contract number 64405).

How we store the data collected
Participation in this questionnaire is completely anonymous. At the end of the study, we will transfer the data from this online survey tool to a secure, password-protected location, after which the data from the online survey tool will be deleted. We may use your anonymous data in publications and other research outputs.

All the data will be stored according to the UK Data Protection Act 1998.

Who will have access to this data
The researchers involved with the collection and the data analysis from the 3D Tune-In consortium will have full access to the data. The findings of the study will be released in the form of a written report to the project’s consortium and will be disseminated as mentioned above. All data will be stored for a minimum of seven years post-publication. After this time, electronic files will be deleted and any hard copies will be destroyed. Nerlaska (a 3D Tune-In consortium partner) is responsible for data storage.

If you understand and agree to these conditions, please continue to the questionnaire by clicking ‘Next’. Thank you very much for taking the time to take part in our study.
