This form will be available until May 14, 2017 at 11:59 pm. Any responses after this period will not be accepted. This form is to re-register your current locker for the 2017-2018 school year and not to re-register a student organization locker. All inputs are required and any errors could jeopardize your locker from being re-registered.
If your locker does not have a lock already on it, you must place a lock on your locker by May 14, 2017 and it must be kept on your locker for the entire summer. Any locker that does not have a lock on it by this day will be given to another student.
