Teacher Request Form (for teachers, day care providers, and home schooling parents)
Use this form to request items for your classroom lessons. We have a high level of familiarity with our collection and will put materials on hold for you, saving you time and energy!

Please give staff at least a week to fill your request. Giving us that amount of time will allow staff to collect materials from other libraries, if needed. If you know your themes well in advance you can fill out your forms well in advance, too!

Tell us when you want to pick them up and they will be ready on the hold shelf for you to pick up and check out. Your holds will show up on your library account, and you will be notified when your materials are ready for pickup.
