Team Checklist Form for Return to Play Guidelines
All USQ teams wishing to hold in-person activities and events while the return to play guidelines for COVID-19 are in effect must submit this form. If as of October 5 your team has already begun practicing in-person, you must submit this form by October 21.
For all other teams, the form must be submitted no later than a week before your team’s first in-person activity or event.
