GCC DE Addendum
Please complete this form for any course requiring a DE Addendum that may be offered in any of our forms of Distance Education at GCC. Do not use this form if your course will only be offered in the Distance Education modality in emergency situations (for example, Study Abroad field studies). This form requires information about the course that fulfills state and federal requirements for DE instruction.

A Course DE Addendum will expire when its COR expires; revised DE Addenda will also be required when substantial changes are made to a COR.
