Denver Sailing Association's Participant Health Self Screening Form
Each participant is required to self-screen for COVID-19 symptoms before each Race day, before they board their boat including taking their own temperature.
IF YOU OR ANY MEMBER OF YOUR HOUSEHOLD are experiencing any symptoms of COVID-19, DO NOT COME TO CHERRY CREEK STATE PARK (THE LAKE) TO RACE. Notify your skipper and the club, isolate yourself from other household members and call your medical provider. Symptoms of COVID-19 include: Cough, Shortness of Breath or difficulty breathing, Fever (100.4°F+), Chills, Muscle pain, Sore throat, New loss of taste or smell. This list is not all possible symptoms. Other less common symptoms have been reported to include gastrointestinal symptoms like nausea, vomiting or diarrhea.
If a participant reports symptoms while at Lake, the Club will:
1. Send the participant home immediately and refer for COVID-19 testing
2. Increase cleaning in the areas where the participant has been and promote social distancing (participants at least 6 feet apart from one another or masked).
3. Exclude participant until they are fever-free (without medication) for 72 hours and 10 days have passed since their first symptom.
4. If multiple participants have these symptoms, contact the Marina and Public Health.

I will complete self-screening prior to each race day and acknowledge that I do NOT have any COVID-19 symptoms. If I develop symptoms after arriving at the Lake, any race day, I will report it to my skipper/crew and Race Committee and leave the Marina. **ENTERING MY NAME CONFIRMS THAT I DO NOT HAVE ANY COVID-19 SYMPTOMS AND THAT I'LL REPORT THEM IF THEY DEVELOP**
