Only PARENTS should fill out this form to excuse absences, unless the student is 18 years old. To report a student absence, please complete the following attendance form. Please enter the PARENT email address in the email address field.

If you have filled out this form to report an absence, please do not email or call. Filling out this form is the preferred method of absence reporting.

A copy of your responses will be sent to your PARENT email address as a confirmation. If you do not receive the confirmation, please check your Spam folder.
