Students: To register for an internship you must complete this form and then forward the email you receive to your Internship Representative who will be supervising your work. They must then forward the application email to Domenic J. Licata <djlicata@buffalo.edu> indicating their consent. Upon the approval of your Faculty Sponsor, you must ask to be force registered into the appropriate Internship course and section. (Three credit hours requires a total of 120 hours of work time to complete by the end of the semester. Two credit hours may be obtained with a total of 80 hours of work time.)
