Gateway Rush and Reiniger Memorial Soccer Park does not carry nor participate in any insurance contract whether for group or individually. No medical insurance of any type is provided. Parent/Guardian states and agrees that the minor child is adequately covered by insurance elsewhere and imposes no fault on Gateway Rush, Reiniger Memorial Soccer Park, the board of directors, the officers, trainers, coaches or any other individuals. The undersigned Parent/Guardian on behalf of himself, the minor child and the minor child’s family agrees to indemnify and hold harmless Gateway Rush, the leagues, Reiniger Memorial Soccer Park, and associations with which the players belong or subscribe, the board of directors, the officers, trainers, coaches, and any other individuals including those who transport them to and from tryouts, practices, and games for any damages or injuries to the child as a result of his/her participation in Gateway Rush. The undersigned Parent/Guardian on behalf of himself, the minor child and the minor child’s family agrees not to file any lawsuit or claim in pursuance of any rights they may hereunder.
