Chorus Fest 2023 School Commitment Form

Please fill out this form by September 15th if you plan to participate in PMEA District 3 Chorus Fest on November 21, 2023 @ Punxsutawney HS. The registration cost will be between $22 and $25 per student. The deadline for submitting student names is October 6 and the deadline for medical forms is October 16th. Folders will be sent digitally to participating schools on September 18th along with fest paperwork.
