Describe in detail the personal activities you participate in to support your Czech/Slovak heritage, including the length of time spent in each activity, the duties you may have had and what you learned from these activities. *
Give a brief overview of your Czech/Slovak ancestry. Include names of family members, what you know about your family history, and other important family information you would like the Scholarship Committee to know. *
Provide a biographical statement about yourself and your chosen course of study.  Why is your field of study important to you?  Why did you choose to study this field? What do you hope to learn from your course of study?. *
Give an overview of the extracurricular activities you participate in, both inside and outside of school. Any special positions you currently hold/have held, any awards you have earned, and other special honors you have achieved from these activities.  *
This application requires 2 recommendation letters by professors, employers, or other professional type references.  It needs to be emailed directly from the person to:  czechheritagescholarship@gmail.com. *
We need you to email us a current PHOTO of yourself. Please do not use Copyrighted photos. *
